Starting point questionnaire Birth – 20 months Please fill in this questionnaire to enable practitioners to get a clear understanding of your child’s developmental starting point and to enable your child’s key person to plan appropriate next steps. The questionnaire is split into the three prime areas of learning (as shown in the EFYS framework.) Please be as accurate as possible when answering the questions.
