ROLE DESCRIPTION
Role Title: Home from Hospital Link Volunteer
Ref: FTC – HHLV
Purpose of Position: Food Train is an award winning charity providing services to older people who are no longer able to manage independently, through age, ill health, frailty or disability. Food Train offers a grocery delivery service, telephone befriending, face to face befriending, home support and meal delivery.
Reports to: Food Train Connects team.
Location: Forth Valley
The Role:
To visit an older person in your community who has been discharged from hospital within 24 hours. They will have received a one day Food Train 'provisions pack'.
To call them to arrange your visit and ask if they require any further emergency grocery items.
Purchase items and take along to their visit. (you will be reimbursed within 24 hours)
Ensure they are aware of all Food Train services that are available to them and answer any questions they may have.
Complete an ‘introduction form’ to gather information for the Food Train Connects team to enable them to allocate the required services.
To report any matters of concern to Food Train staff
To inform Food Train Staff of any accidents, incidents or complaints
To maintain confidentiality
To represent Food Train in a professional manner to customers and the wider community
Skills and experience:
There are no formal educational requirements for this role however knowledge of the needs of older people acquired through life and family experiences, training received or practice based experience is desirable.
Friendly, sociable and patient with a caring manner.
Responsive, flexible, efficient and able to respond to visit requests within 24 hours.
Good verbal communication skills are required as well as being a people person with a sense of humour who can adapt to any situation and changing people behaviour.
