  • IMPORTANT INFORMATION FOR ALL CHILDREN AND ADULTS ATTENDING FIRESIDE FOREST SCHOOL EVENTS

    • Please ensure that you arrive promptly.

    • Please ensure that anyone (adult and child) attending sessions has outdoor activity clothes - approprite to season- to include trousers, a long sleeved top and sturdy shoes. Also bring waterproofs and a change of clothes, wellies if wet.  It is often refreshingly cooler under the tree canopy and even on a really sunny day, you might find yourself reaching for long sleeves.

    • Closed toe shoes/wellies only: flips-flops, sandals and crocs don't offer enough protection from brambles, stingers etc. - Anyone with unsuitable foot wear or clothing will be taking part at their own risk.

    • Please bring a hat and gloves in winter/hat and sun protection cream in summer. If wet a change of clothes for the way home.

     

  •  MEDICAL CONSENT FORM

    Please complete this form: all information will be treated in the strictest confidence.

  • By completing this form I am giving consent for my child to take part in outdoor activities and understand that these activities may involve an assessed/measured level of risk and may include, walking, craftwork, woodcraft, including using sharp equipment such as knives, fire lighting skills, den building, campfire cooking and other related activities.

    By completing this form I am giving consent for the equipment and tools necessary for the activity to be used by the young person mentioned above. I understand that a strict code of practise for working with children will be followed and all activities will be risk assessed and I will be informed of any extra details of activities that are out of the ordinary pattern.

    Fireside Forest School cannot take responsibility for any property, which may be lost or damaged during sessions, and therefore we recommend that any mobile phones or valuable items are not brought to the sessions. In an emergency, I understand that every effort will be made to contact me, but I consent to any medical treatment required.

  • Consent form: As parent/guardian of the child named above I agree to my child taking part in the activities and have completed and signed this consent/medical form. I have read, understand and agree to the information at the top of this form. I know of no reason medical or otherwise why they should not take participate. I will ensure the Forest School Leader has any medication i.e. inhalers/epi-pens at the start of the session. I consent to any emergency treatment necessary during the above session. I therefore authorise Fireside Staff to sign on my behalf any written form of consent required by the hospital authorities on the understanding that every effort will be made by the Fireside staff to contact myself the parent/guardian on the numbers provided above. I accept that my child will probably get muddy and will ensure my child and any adults accompanying are dressed appropriately, with sturdy shoes, long sleeved top, trousers, waterproofs etc. I understand that even though the activities are well managed and risk assessed, they can still carry a degree of risk and potential danger. I understand that my child or accompanying adults will be excluded from taking part/accompanying the group if appropriate clothing/shoes are not worn.

  • Fireside personal data (including name, address, email, and telephone number) of existing customers, participating children, and school contacts on the basis of contractual relationships, and data of potential customers on the basis of our legitimate interest in providing educational woodland activities. Fireside customers will be deemed to have consented to essential communication by email and phone as part of the contractual relationship. 

    Your data is not passed on to third parties without your permission. Your data is stored electronically within our booking system and on paper consent forms, in compliance with the GDPR. Cloud servers storing the data may be located in a destination outside the European Economic Area (EEA) but the hosts are committed to GDPR compliance. If you object to this transfer, storing or processing of your data, please contact us.

  • Photography Consent

  • Fireside Forest School use photographs taken during Forest Adventure Parties to share ONLY with parents/guardians of those participating. We may from time to time, use them on our website and our social media pages for promotional and publicity purposes. This consent form will cover all activities.

  • Indeminity Statement

  • I declare that I have read and agree to the content of this document. I have discussed the content with my child/ren and made them aware of their responsibilities.

  • Taking part in outdoor learning and Forest School activities is conditional on individuals recognising that these activities are activities where the potential for misadventure exists, including personal injury. Participants shall be aware of and accept these risks and agree to be responsible for their own actions and involvement. I declare that in the event of any claim of compensation for an incident, the claim will only be against another party directly involved in that incident. No compensation will be sought from the Company or directors unless they are found, by law, to have acted in a negligent or illegal manner. It must be remembered that safety is the responsibility of ALL participants who are each responsible for the safety of themselves and other participants. All safety rules and procedures must be followed. Any hazards found in / on the site must be reported to the Forest School Leader/assistant who will take all reasonable steps to ensure every participant is made aware of the hazard.
