Magical Christmas Mini Session Booking Form
This form can be used to book and pay for a Magical Christmas Mini Session or if you prefer I can take your booking over the phone.

These sessions include:

30 minute session in the studio

A gift for every child from Santa

For up to 5 immediate family members (Parents, children, Siblings)

3 beautifully edited digital images consiting of one Christmas photo and two classic portraits.

Suitable from newborn upwards (As long as someone involved in the photo shoot is able to confidently hold the newborn)

Your 3 images will be sent to you via email within 1-3 weeks of the session.

Sessions available on the below dates only so this is your only chance to book one this year.

I recommend that pyjamas are worn for the Christmas photo, these don't have to be Christmas themed, or Christmas jumpers with black leggings/jeans/cords or dark blue leggings/jeans/cords.

A second outfit can be changed in to for the two portraits after. Please try to wear plain tops and bottoms to avoid any distractions in these portraits.

I, do hereby grant/deny permission to Angel Eyes Photography to use the images from my photo shoot, as marked by my selection(s) below. Such use includes the display, distribution, publication, transmission, or other use of photographs and images taken of my child for use in materials that include, but may not be limited to, printed materials such as brochures and newsletters, videos, and digital images such as those on the Angel Eyes Photography Web site and Facebook page.

Please note that nothing will be shown on my Facebook page, website or any other site until after Christmas day as long as you let me know by selcting the option below on this form.

Unrestricted usage: I give unrestricted permission for the images to be used in print, video, and digital media. I agree that these images may be used by Angel Eyes Photography for a variety of purposes and that these images may be used without further notification. Such as printed materials, samples in the studio, online use and competition entries, online and printed publications. I do understand that our surnames will not be used in conjunction with any video or digital images. Limited online usage: Images can be used on Angel Eyes Photography's Facebook page and website Limited usage: I wish for the images to be used within the Angel Eyes Photography studio setting only. Deny permission to use any of the images at all.

Please note that no refunds are given for these sessions. You may reschedule to another date and/or time ONLY if you give me a minimum of 7 days notice via phone call and if there are more sessions avaiable to choose from. If you fail to give my the required 7 days notice then you will not be able to reschedule. If there are no other suitable slots then you will not be able to reschedule. Sorry for any inconvenience this causes but due to the level of planning and work needed for these I can only run them for one week.

During the photo shoot I understand that any children in my care are my responsibility. I will not allow them to do any of the following:

Run around in the studio

Touch any of the photographic equipment especially the background paper

Eat food in the studio

Or anything else that could put themselves or others in danger or risk damage to the equipment

I understand that any breakages must be paid for by me in full at the appropriate rate and no images will be released until any outstanding amount has been paid.

I also understand that Angel Eyes Photography reserve the right to terminate the photo shoot at any time if it is felt that anyone including the photographer is placed in any danger or any breakages have or are likely to occur.

No footwear is to be worn on the background paper please.

The photographer is under no obligation to display any of the images during the photo shoot.

The photographer will provide colour corrected and edited images for previewing. I understand that the final content of the images are at the discretion of the photographer and there is no right to reject the images because of style or composition. Not every image that is taken at the photo shoot will be included in the gallery and the photographers decision is final.

Under no circumstances is the copyright released and RAW files are never included for sale.

No shots are completely guaranteed.

Angel Eyes Photography will take every possible care and use every working method available to prevent damage or loss to any of or all of the images taken on the day but in the unlikely event that any or all images are lost because of mechanical failure, theft, or any other way, liability is limited to either a re shoot subject to availability or return of fees paid for the photo shoot either in part or in full.

By paying the upfront session fee you are agreeing to the above.

Please note that all copyrights of the images remain with the photographer.

Angel Eyes Photography cannot be held responsible for any accidents or injuries that occur during the photo shoot.
