KEY fob activation

This Key fob activation form is only to be filled in by Lieven de Key employees. It serves both to order a key fob as a contract with the keyfob user.

Key fob user hereby confirms to have received a key fob for their apartment. He/she is aware that loss or breakage of this key fob can result in the replacement fee of €150,-.

At the end of agreed usage term, user needs to return the key fob with charger cable in good condition or can be charged €150,- replacement fee.

The key fob needs to be handed to the building caretaker or placed in their mailbox.

A copy of this form will be sent to the key fob user, by accepting the key fob the user agrees to these terms.
