Now, coming back to your question, when I cloned your form and tested it, I saw that you used a Page Break element to divide the form into 2 pages. That's why, after your customers select a product, they need to click on Next button. You can use Skip to Page condition to skipping any page in the forms which contain multiple pages, but you have 2 pages in your form so you don't need to use any conditions to skip a page.

I saw that you used the feature Redirect to an external link after submission on your form, and you can use this feature for the appointment form as well. You can add your website URL into the second form's Thank You Page. First, you need to create a second form which contains appointment information. Then, once someone finishes the first form, they'll be automatically directed to the second form. Let me show you how to set that up: 

1. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
2. In the menu that opens on the left side of the page, click on Thank You Page.
3. Now, select Redirect to External Link and paste the URL of the second form.
