I was able to confirm that the 6152061343112360291.pdf actually comes from the Signature field on your form. You can toggle OFF the Enable Jotform Sign Automation field on Jotform Sign field in Smart PDF Forms. You can clone my demo form, or you can do it on your end. Let me show you how to do it:

1. In Smart PDF Forms, click on Settings on the blue navigation bar.
2. Click on the Jotform Sign tab on the left part of the screen.
3. Toggle OFF the Enable Jotform Sign Automation field on Jotform Sign field.
