2021 ACTIVITY MEMBERSHIP FORM
DUE TO COVID-19 restrictions and to enable us to monitor the number of customers on site at any one time, it has become neccessary to register prior to attending activity sessions at ACTIVITIES AWAY.
Our MAXIMUM ACTIVITY GROUP SIZE is 6 people
ONLY the lead person named on the booking needs to complete a form as you will add the names of everyone else in your group on a later screen.
All data provided is deleted within a month of your activity session.
You will need to complete a form each time you come to ensure we have the correct data recorded for the day you attended.
It will take a few minutes to work through the form and to AGREE OUR RULES & RECOMMENDATIONS for use of the facilities.
Completion and signing this form is a CONDITION OF USE and must be agreed prior to membership acceptance.
If you cannot agree to the RULES & RECOMMENDATIONS then you must abandon your application and contact us using email@example.com to cancel your booking.
PLEASE NOTE - you MAY experience some 'lag' while completing the form, this is normal as our database is updating in the background and is affected by things like number of users and the available bandwith or speed of your internet connection.
If you just wait a few seconds it should catch up and you will be able to complete the process.
