When I've checked your form, there is no current integration added on it. Check out my screenshot below:

When I clicked on Google Sheets, it appears that Google Integration is disabled. Check out my screenshot below:You have to re-authenticate your Google Sheets account for it to be reintegrated. Let me show you how to do this:

1. In Form Builder, click on Settings on the orange navigation bar at the top of the page.
2. Select Integrations from the left side window.
3. Click on Google Sheets.
4. Click on Re-Authenticate at the bottom of the page and add back your Google Sheets account.
