Problème avec le widget "Spreadsheet to Form"

Je suis confronté à un gros problème depuis quelques semaines.

Lorsque j'utilise le widget "Spreadsheet to Form", des éléments de mon formulaire disparaissent ce qui est très ennuyeux pour la suite de mon processus.

Pourriez-vous m'expliquer pourquoi mon NOM et Prénom disparait une fois le widget a été utilisé ?

J'espère que vous trouverez une explication car cela est problématique pour toute l'équipe.

Now, coming back to your question, can you check if the column names in your spreadsheet match the field labels on your form exactly? Also, verify that the unique code used in the widget matches the unique code in your spreadsheet.

Je vous confirme que les champs correspondent parfaitement aux noms des colonnes de mon tableau Excel.
Les valeurs de champs qui disparaissent n’ont aucuns liens avec le widget et le tableau Excel.
