Parents Permission Slip Template
Dear parents or guardians,
The approval of absences from school requires the signature of both the student's parent or guardian and their principal or guidance counselor on the permission slip. All students attending any of events are required to have a permission slip that has been fully completed.
By signing this agreement you agree with the following statements,
- I certify that I am the legal guardian of the child named above, and I have read and understand the contents of this agreement.
- I give permission for my child to participate the event.
- I understand that the school will take reasonable precautions to ensure my child's safety during this activity, but I acknowledge that there are risks involved in such activities.
- I also agree to release the school, its employees, agents, and representatives from any and all claims, damages, and causes of action that may arise from my child's participation in the activity or event.
- I also authorize the school to seek emergency medical treatment for my child if necessary.
