Hola,
Tengo un inconveniente con el ingreso y uso al siguiente formulario: https://www.jotform.com/app/231454827620152
Al ingresar se genera el siguiente mensaje:
Me pasó durante 10 ocasiones en la mañana del día de hoy, quisiera saber el motivo real ya que este formulario es muy importante para la compañía en la que trabajo y que no funcione genera horas de parada en uno de los procesos.
De este modo, encontrar la solución cuando suceda lo mismo en siguientes oportunidades.

Our Spanish Support agents are busy helping other Jotform users right now, so I'll try to help you in English using Google Translate, but you can reply in whichever language you feel comfortable using. Or, if you'd rather have support in Spanish, let us know and we can have them do that. But, it might take a while until they're available again.
Now, coming back to your question, I cloned your app and tested it to see if I could replicate the issue, but everything was working properly. Check out the screencast below to see my results:
It could be a temporary issue and seems to be working as it should. I have cleared your form cache just in case.
