Envíe un formulario con respuestas precompletadas, solo muestra un campo para seleccionar

Tengo este formulario que en el apartado de párrafo cuando utilizo el botón de enviar formulario con respuesta pre completada

solamente me parece un solo campo

About the issue, Fill in the blank element isn't supported when using prefill. Tables Prefill supports the following elements:

• Full Name
• Email
• Address
• Short Text
• Long Text
• Phone
• Number
• Dropdown
• Single Choice
• Multiple Choice
• Scale Rating
• Spinner
• Date Picker

You may need to redo the form and use elements that are compatible with Table Prefill.
