I cloned your form and tested it to see if I could replicate the issue, but everything was working properly. Check out the screencast below to see my result:
Like what you see in the above screencast, I was able to upload a JPG file. I've cleared the cache of your forms, can you try again? If the issue persists, can you try to use a different browser or connect to a different internet service provider?
