Bonjour,
Je souhaiterais ajouter un fichier Excel lié à un de nos jotforms satisfaction client.
L'objectif est que le client tape son nom et son nom de famille dans le formulaire et que son adresse mail et numéro client s'affiche automatiquement
Comment puis-je faire ?

Or, if you'd rather have support in French, let us know and we can have them do that. But, it might take a while until they're available again.
Now, coming back to your question, you can use the Spreadsheet to Form widget with Update/Calculate Field condition. Here's how to do it:
How To Add Spreadsheet To Form Widget:
1. In Form Builder, on the left side of the screen, click on Add Element.
2. On the left panel, go to Widgets.
3. Search for Spreadsheet to Form.
4. Click/Drag to add Spreadsheet to Form into the form.
How To Configure Spreadsheet To Form Widget:
1. In Form Builder, click on Spreadsheet To Form to select.
2. On the right side of the selected Spreadsheet To Form, click on the Wand icon.
3. On the right panel, click on Upload File and select your Excel file.
4. Set Access code column to use the name column on your Excel file.
5. Scroll down to Autofill By Condition and set it to Yes.
6. Scroll down to Run in the background and set it to Yes.
7. Click on Update Widget.

How To Create Update/Calculate Field Condition:
1. In Form Builder, in the orange navigation bar at the top of the screen, click on Settings.
2. Click on Conditions on the left side of the screen.
3. Select Update/Calculate Field.
4. Configure the condition as shown in the screenshot.
5. Click on Save.
