Uploading PDF file to existing form

Bonjour,

Je souhaite transformer mon formulaire "Demande remboursement frais suite lésion professionnelle" en formulaire pdf intelligent afin d'uploader le rendu pdf que je souhaite et positionner les champs de formulaire sur celui-ci. Je souhaite m'éviter de refaire manuellement mon formulaire "Demande remboursement frais suite lésion professionnelle" dans l'éditeur de pdf intelligent.

Now, coming back to your question, unfortunately, it is not possible to Upload PDF documents after you have created the form. If you have an existing PDF document file, you should use the Smart PDF Form. By using Smart PDF Form you can turn an already existing PDF into a fillable form. It will also create a form based on your PDF file so you won't need to manually create all the field, you can also modify the form so you can check if the form matches the PDF file correctly. Let me show you how:

1. Go to My Forms and select Create Form in the upper-left corner of your screen.
2. Under Create a Form, select Import Form.
3. Select Import PDF form to open Smart PDF Forms.
4. In Smart PDF Forms, select Upload PDF Form, then choose your PDF file.

Once that’s done, you should see your imported PDF along with the online form created from it. Note that the online form will not look the same as the imported file. The system will try its best to fetch and clone the form fields from your PDF file.
