Requets to lift suspension on the account

If you agree to follow our Terms of Use and let us delete the offending fields mentioned above, we can go ahead and lift the suspension on your account.

Once we hear back from you, we'll be able to move forward with this.

I have removed the offending fields and reactivated your account. I have also cleared your form cache to clear any errors on the form. You can now log in to the Jotform account.
