Bonjour,
Mon compte à été suspendu, or je ne vois pas pour quelle raison. c'est un formulaire de santé que j'ai créer pour mes clients. Dans ce questionnaire à aucun moment je ne demande d'informations tels les coordonnées bancaire, permis, carte identité ...
est ce possible de réactiver mon compte ?

Now, coming back to your question, Your account was suspended because you’ve got a form that was tagged as spam/phishing. We understand that this might not be the case with your form, but we had to act according to the general rule to be in compliance with our Terms of Use.
I did check your form, and it looks like the tagging was a false positive, so I reactivated your account. You should be able to log in now.
