All forms created in Jotform are designed to be responsive for all devices, including mobile phone and tablet. I have created a form for you from scratch that is responsive on mobile and tablet. Take note that since mobile phone has small screen, you can expect that some fields are adjusted. You can use or check the form I created for you here by cloning it.
