Now, coming back to your question, can you please tell us more about what issue occurred with your form? I cloned your form and tested it to see if I could replicate the issue, but everything was working properly. Check out the screencast below to see my results:
Can you please test the form again and let us know if the issue persists? I have also cleared your form cache in the meantime. If the issue persists, will you allow us to create a test submission in your form directly?
