Now, coming back to your question, your account was suspended because you’ve got a form named Permição de adicionar colaborador no sistema, that’s breaking our Terms of Use by collecting login credentials.
If you agree to follow our Terms of Use and let us delete the offending fields mentioned above, we can go ahead and lift the suspension on your account.
Once we hear back from you, we'll be able to move forward with this.

would violate jotfrom guidelines.
I created a form and the purpose of this field (password) is just password
suggestion, but now I understand how risky this can be, so I agree to
delete the field that violates guidelines, and thanks again for being part
of an amazing platform like jotfrom.
