Bom dia,

Hi Welenson,
Thanks for reaching out to Jotform Support. Unfortunately, our Portuguese Support agents are busy helping other Jotform users at the moment. I'll try to help you in English using Google Translate, but you can reply in whichever language you feel comfortable using. Or, if you'd rather have support in Portuguese, let us know and we can have them do that. But, keep in mind that you'd have to wait until they're available again.
As for your question, I’ll need a bit of time to look into this. I’ll get back to you as soon as I can.

Hi Welenson,
Yes, you can use the Spreadsheet to Form widget to import data and use it for conditional logic to create an input field that will search for the names and return the information of the searched name to the form field. Check out the screencast below:
As you can see, the information for Age and Gender appears after typing the Name from the Spreadsheet. You can clone my demo form, or you can do it on your end. Let me show you how to do it:
1. In Tables, click on Download All on the upper right part of the screen.
2. Click on Download as Excel.3. Go to Form Builder, click on Add Form Element on the upper left part of the screen.
4. In Form Elements, scroll down.
5. Drag and drop Short Text element on your form.6. Change the label of the Short Text field to "NameList".
7. Click on the Widgets tab on the upper left part of the screen.
8. Search for "Spreadsheet to Form".
9. Drag and drop Spreadsheet to Form on your form.10. Click on the Wand icon of your Spreadsheet to Form widget, and click on Upload File.
11. Search and click on the downloaded Excel file.
12. Click on Open.13. In the Widget Settings, click on Access Code Column field.
14. Click on "NameList".15. Click on Autofill By Condition field.
16. Click on Yes.17. Click on Run in the Background field.
18. Click on Yes.
19. Click on Update Widget.20. While holding Ctrl on your keyboard and click on NameList field.
21. Click on Age field.
22. Click on Gender field.
23. Click on Submit field.
24. Then, click on Hide on the orange navigation bar.25. Click on Settings on the orange navigation bar.
26. Click on Conditions on the left part of the screen.
27. Click on Show/Hide Field.
