The "Type a Label" field is a supplemental response to the Multiple Choice ?

My colleague used the Fill in the Blank field. Let me show you how to set this up:

1. In Form Builder, click on Add Element on the left side of the page.
2. From the Basic tab, select Fill in the Blank.
3. Remove the default fields.
4. Click on Add field and select Multiple Choice and Short Text.
5. To go to the next line, just click on enter.

The Multiple-choice fields and the text fields are not related to each other. However, as there is a text field next to every Multiple Choice, it will appear like this in the submission tables. Here is a screenshot:

Moreover, you can add the label for each text field so they look more relevant in the submission tables.

1. In the Form Builder, click on Short Text Field Properties in the Field.
2. Now, type the label.
