Ao final, a 1ª coluna (Autoavaliação) terá uma totalização. Da mesma forma, a 2ª coluna. Grato

Thanks for reaching out to Jotform Support. If you want to add your data on Jotform in the Table form, you can either use Input Table field or you can add the Spreadsheet widget. Let me show you one by one:

Input Table

1.  In Form Builder, click on the Add Element menu on the left side of the page.
2. Under the Basic tab, scroll down the list and choose the Input Table.
3. Now, just drag and drop it to your form.
4. Select the Input Table, click on the Gear icon, and you can modify the Field types under Fields tab.

Spreadsheet

1. In Form Builder, click on the Add Element menu on the left side of the page.
2. Under the Widgets tab, enter Spreadsheet in the search bar, and drag and drop the widget to your form.
3. Select the Spreadsheet and click on the Wand icon.
4. Under General tab, you can customize Rows and Columns & click on Update button.
