Boa tarde,
Quero criar um teste de produtividade, serão 10 perguntas, cada uma com a possibilidade 5 respostas, segue um exemplo:
EX: Você tem disciplina para manter sua produtividade mesmo nos dias difíceis?
Resposta:
Sempre → 5 pontos
Frequentemente → 4 pontos
Às vezes → 3 pontos
Raramente → 2 pontos
Nunca → 1 ponto
No final das 10 perguntas, terão algumas possibilidades de respostas para o teste:
40 a 50 pontos → Alta Performance e Produtividade Máxima
30 a 39 pontos → Produtiva, mas com Melhorias a Fazer
20 a 29 pontos → Produtividade Instável e Oscilante
Menos de 20 pontos → Baixa Produtividade e Bloqueios
Gostaria de fazer isso de forma automática.
Após a pessoa preencher o teste, fazer a somatória das respostas e enviar o resultado do teste para o email da pessoa (iria configurar a resposta detalhada para cada pontuação)
É possível realizar isso com o jotform? Teria algum tutorial para assistir que e ensine a realizar?

Hi Tatiane,
You can achieve this by assigning Calculation Values to the answers or options, then, use Conditional Logic to send Autoresponder Emails to form submitters according to their total score. I can walk you through it step by step. But note that to completely achieve this, you'll need to be able to create multiple Autoresponder Emails. Autoresponder Emails are those emails that are sent to the form fillers after they submit your form. Now, since you're currently under our Starter (Free) plan, you're unable to add multiple Autoresponder Emails yet.
You'll need to Upgrade your Account to any of our paid plans. Let me show you how:
1. On your My Forms page, click on Pricing in the blue navigation bar at the top of the page.
2. On the Pricing page, choose the plan you want to buy.
3. In the popup window, choose either a Monthly or Yearly payment schedule and either Credit/Debit Card or PayPal as the payment method.
Here’s an example of the Bronze plan:
After you click on Continue, you'll need to enter your Credit/Debit Card details.
If you choose to use PayPal, you’ll get redirected to the PayPal payment page to complete the payment. That's it. Once you're done upgrading, here's how you can achieve what you want to do:
1. First, let's assign Calculation Values to the options:
a. In Form Builder, click on the Question field's Gear icon to open the Properties panel.
b. Go to the Options tab, and toggle Calculation Values to On.
c. Enter the value of each option, equivalent to their scores.
d. Do the same for the rest of the questions.
2. Now, let's add a Form Calculation widget to your form, which will calculate the total score:
a. While in Form Builder, click on Add Element in the upper-left corner of the page.
b. Go to the Widgets tab, and select Form Calculation.
c. On the Widget Settings panel that opens on the right, set the calculation as shown in the screenshot below, and click on Save at the bottom.
3. Now, let's create multiple Autoresponder Emails:
a. Click on Settings in the orange navigation bar at the top part of the page, and select Emails from the menu on the left.
b. Hover your mouse over Autoresponder 1, and click on the Pencil icon to edit it.
c. Under the Email tab, add your customized message or email content and edit the title of the Autoresponder Email at the top for easier identification later.
d. Click on Apply on the right, and on Save at the bottom.
e. To add another Autoresponder Email, click on Add Email at the top, and select Autoresponder Email.
d. Change the Autoresponder Email title, and email content as desired.
e. Click on Save at the bottom.
f. Do the same steps to add the Autoresponder Emails that you need, as shown in the screenshot below:
4. Lastly, let's set up Conditional Logic. This is to sent Autoresponder Emails based on the total score:
a. While in Settings, select Conditions from the left-side menu, and select Change Email Recipient.
b. Set the first set of conditions as shown in the screenshot below, and click on Save at the bottom.
c. To add another set of conditions, click on Add Condition at the top, and select Change Email Recipient.
d. Set the second set of conditions as shown in the screenshot below, and click on Save at the bottom.
e. Add the rest of the conditions. The screenshots below show how they should be set up.
